What it is and how it works

Hydroquinone is a prescription-only depigmenting agent. In the EU the 4% cream and gel are authorised for melasma, chloasma, lentigines and freckles. It slows the formation of new melanin, so its effect is gradual and reversible: pigment returns if sun exposure resumes.

The cream is the standard base; the gel adds glycolic acid in an alcohol vehicle, which some prescribers prefer for resistant patches on oilier skin. A licensed doctor confirms the pattern of pigmentation, sets the course length and reviews at 2 months.

What is hydroquinone 4% used for?
Hydroquinone is a prescription-only depigmenting agent that slows the production of new melanin. In the EU the 4% cream and gel are authorised for melasma, chloasma, lentigines and freckles. Its effect is gradual and temporary: pigment returns if sun protection lapses after the course.
First patch-test on the inner forearm for 24 hours. Then apply a thin layer only to the patches once a day for 10 to 15 days, and twice a day after that if tolerated. The recommended course is 60 to 90 days, and it is never used continuously for more than 6 months. Broad-spectrum SPF 50+ every day is part of the treatment.

Mild burning, redness or dryness at the site is common early on. A halo of lighter skin around the patch can appear if the cream spreads. With very prolonged use, a blue-black darkening called ochronosis can occur, which is why courses are time-limited. Stop and seek review if the skin darkens or the product itself turns dark.
It is not recommended in pregnancy or breastfeeding; melasma of pregnancy is managed with sun protection and reviewed after delivery. Before prescribing, the doctor confirms the pigmentation pattern, asks about hormonal contraception, previous depigmenting products and any skin disease in the area, and sets a review at 2 months.
