Short answer: yes, Ozempic is available in Spanish pharmacies with a prescription — but there is a distinction that changes the answer depending on why you want it. Using Ozempic to lose weight is off-label prescribing. That does not make it impossible — a doctor can prescribe off-label under their own clinical judgement and responsibility — but it is not the default, and it needs a reason specific to your case. Can you buy Ozempic in a Spanish pharmacy?
Yes, with a valid prescription. Semaglutide is prescription-only in Spain and pharmacies are legally required to see a prescription before dispensing, for residents and visitors alike. There is no over-the-counter version, and any site offering one without a prescription is operating outside the law.
Online sale is prohibited outright, not merely discouraged. Royal Legislative Decree 1/2015 bans "the sale, by correspondence and by electronic means, of medicines and medical devices subject to prescription", and Spain's medicines agency restated in September 2025 that prescription medicines containing GLP-1 analogues cannot be sold over the internet in Spain. Unlawful internet sale is classed as a very serious infringement, with fines from €90,001.
Semaglutide is also among the most counterfeited medicines in Europe. In October 2023 the European Medicines Agency alerted patients and healthcare professionals to falsified Ozempic pens in the European supply chain — packs that carried serial numbers copied from genuine ones and were caught only because the serials scanned as inactive. In September 2025 the EMA and the heads of national agencies warned again about the sharp rise in illegal medicines sold in the EU, naming semaglutide, liraglutide and tirzepatide, and noting such products "may not contain the claimed active substance at all and may contain harmful levels of other substances".
How to get a prescription without a clinic visit

One nuance here decides what you actually pay.
Ozempic is listed in Spain's official nomenclátor, so it has a fixed retail price: €117.89 for a single-pen pack (September 2026 nomenclátor; Novo Nordisk cut it by 8% on 1 February 2026). The 1 mg pack with 8 needles is €204.83.
On a public prescription, Ozempic is funded — but more narrowly than most people assume. It needs a visado, and cover is limited to type 2 diabetes in people with a BMI of 30 kg/m² or above and in combination therapy with other antidiabetics, not as monotherapy. The 2 mg weekly dose is excluded from public cover entirely. It is never funded for weight loss.

What semaglutide does, and how long it takes
Semaglutide mimics GLP-1, a hormone the gut releases when you eat. It slows gastric emptying so fullness lasts longer, acts on appetite regulation, and improves glucose control. The practical effect is eating less without fighting hunger all day.
What it does not do: burn fat on its own, replace changes to diet and activity, or produce results in a week. The dose escalates in steps over months so the digestive system tolerates it. If treatment stops without anything else having changed, weight tends to return.
Side effects and when to stop
The common ones are digestive and cluster around starting treatment or stepping up a dose: nausea, diarrhoea, constipation, vomiting, abdominal pain. These are usually mild and settle.
Some signals mean stopping and seeking medical attention without waiting: severe persistent abdominal pain radiating to the back, vomiting that will not stop, signs of an allergic reaction (swelling of the face, lips or tongue, difficulty breathing), or sudden vision changes if you have diabetic retinopathy. If you also take insulin or a sulfonylurea, watch for hypoglycaemia symptoms.

Using a prescription across borders

Directive 2011/24/EU requires each Member State to recognise a prescription issued in another Member State for a medicine authorised on its territory, and the pharmacist dispenses the locally marketed equivalent of the same active ingredient. Recognition is not automatic or guaranteed — the pharmacist keeps the right to refuse.
