What to know before your consultation
Obesity and the options doctors use
Obesity is a long-term condition in which excess body weight raises the risk of problems such as diabetes, high blood pressure, sleep apnoea and joint pain. Healthier eating and more activity remain the foundation, and for some adults a prescription medicine can help alongside them. The options a doctor may consider through Hi Doctor are weekly injections (Mounjaro, which contains tirzepatide, or Wegovy, which contains semaglutide), daily Saxenda injections (liraglutide) and Orlistat capsules. The doctor decides which one, if any, fits your health and preferences.
Who online weight care suits
The consultation is for adults with a BMI of 30 or more, or 27 or more with a weight-related condition such as high blood pressure, prediabetes or sleep apnoea, in line with the EU product information. These medicines are not used during pregnancy or breastfeeding, or if you are trying to conceive. A history of pancreatitis, an eating disorder or several complex conditions may mean you need in-person care, and in that case the doctor tells you and your consultation fee is refunded. The service does not replace emergency care: for urgent symptoms, call 112.
Side effects and warning signs
The most common side effects of the injections and tablets are nausea, diarrhoea, vomiting and constipation, usually mild and mostly while the dose is being increased. Less common but serious risks include inflammation of the pancreas, gallstones, dehydration that can strain the kidneys, and low blood sugar if you also take insulin or a sulfonylurea. Orlistat mainly causes oily, loose stools and can reduce the absorption of some vitamins. Severe abdominal pain that does not settle, pain spreading to the back, yellowing of the skin or eyes, or signs of severe dehydration need urgent care: call 112.
Other routes to a healthier weight
Healthier eating, physical activity and behavioural support are the first step for everyone and stay important during any treatment. Some people are better served by a local weight-management service, a dietitian, or a specialist assessment for bariatric surgery, which needs in-person care. If the doctor decides that medicine is not appropriate for you, they explain why in the private chat, suggest what to do next, and your consultation fee is refunded. Choosing not to start medicine is always a valid decision.
Weight-loss research and approvals
The European Medicines Agency has authorised Wegovy, Mounjaro, Saxenda and Xenical (orlistat) for weight management in adults, alongside diet and physical activity. In the STEP 1 trial, adults with overweight or obesity and without diabetes lost 14.9% of their body weight on average with semaglutide 2.4 mg over 68 weeks, compared with 2.4% on placebo. In SURMOUNT-1, adults on the highest tirzepatide dose lost 20.9% on average over 72 weeks, compared with 3.1% on placebo. In the SELECT trial, semaglutide lowered the risk of heart attack, stroke and cardiovascular death in people with overweight or obesity and existing heart disease. These are group averages, not a prediction for any one person.
How your weight consultation works
You start with an online questionnaire about your weight, height, health history and any medicines you take. An EU-licensed doctor reads your answers, may ask follow-up questions in the private chat in your account, and decides whether a prescription is appropriate. If it is, you receive an e-prescription and collect the medicine at a pharmacy of your choice, where you pay for it separately. Prescriptions are valid at pharmacies across the EU, although in some countries the pharmacist dispenses the local equivalent of the same active ingredient.
Reviews, renewals and stopping
Obesity is a long-term condition, so treatment often continues for many months when it is working and well tolerated. Before each renewal your doctor reviews your weight, side effects and dose, and changes are never automatic. Weight tends to return after stopping, so any decision to stop or step down is planned with your doctor.
