How to use
- What Xenical does
Orlistat acts in the gut by blocking some enzymes that digest dietary fat, so part of that fat is not absorbed. It does not suppress appetite and is intended to be used with a nutritionally balanced, moderately reduced-calorie diet.
- Take one capsule with a fat-containing main meal
Take the prescribed 120 mg capsule immediately before, during, or up to one hour after a main meal containing fat, up to three times daily. Swallow it with water. If a meal is missed or contains no fat, skip that dose; do not take it later or double the next dose.
- Review medicines and vitamins
Tell the prescriber if you use ciclosporin, warfarin or another anticoagulant, levothyroxine, epilepsy medicines, amiodarone, or HIV medicines. Orlistat can reduce absorption of vitamins A, D, E, and K; take any recommended multivitamin at least two hours after orlistat, commonly at bedtime.
Dose schedule

Distribute dietary fat across three main meals rather than concentrating it in one meal. Higher-fat meals make oily stools, urgency, and leakage more likely. Keep fluids, fibre, protein, and vegetables in the plan agreed with your clinician.
- At review
The clinician reviews weight trend, digestive tolerance, nutrition, other medicines, and whether treatment remains appropriate. Do not continue automatically if there is no useful response or side effects are difficult to manage.
Side effects
Digestive effects
Oily or fatty stools, gas with discharge, urgency, more frequent bowel movements, and leakage are commonest early in treatment and after high-fat meals. Reducing the fat in each meal usually helps. Contact the clinician if symptoms are severe, persistent, or causing dehydration.
Uncommon but important problems
Seek prompt medical advice for severe or persistent abdominal pain, vomiting, yellow skin or eyes, dark urine, pale stools, unusual bleeding, reduced urine, or back pain with blood in the urine. These can signal liver, pancreas, kidney, gallbladder, or clotting problems.
Do not use if
- Chronic malabsorption syndrome or cholestasis
- Breastfeeding; do not start during pregnancy or while trying to become pregnant
- Allergy to orlistat or any capsule ingredient
When to contact your doctor
- Severe abdominal pain, breathing difficulty, facial or throat swelling, collapse, or a widespread blistering rash: seek urgent care
- Digestive effects that remain difficult to manage despite reducing meal fat
- A new medicine is started, especially an anticoagulant, ciclosporin, levothyroxine, or an epilepsy medicine
Storage
Store in the original package, protected from moisture, and out of children’s reach. Keep the container tightly closed and follow the temperature limit and expiry date on the pack.
What to expect
Orlistat works only with meals that contain fat, and digestive effects reflect how much fat reaches the gut. Weight change is gradual and varies. The clinician reviews whether the medicine is helping and remains safe; nutrition, movement, sleep, and follow-up remain part of the treatment.
