Short answer: Viagra is available in Spanish pharmacies, but only with a prescription, and you pay for it yourself. You do not need a urologist and you do not need to wait weeks for an appointment — any licensed doctor can assess erectile dysfunction, and that assessment can happen online.

If you live in Spain, are moving there, or are spending a long stretch there, this page covers what matters: who can prescribe it, why the generic costs a fraction of the branded box, what the doctor checks before approving, and why buying without a prescription is a worse idea here than with almost any other medicine.

Who can prescribe Viagra in Spain?

Any licensed doctor — a specialist referral is not required. Erectile dysfunction is routinely assessed in general practice: what the doctor needs is your medical history, your current medication and your cardiovascular risk factors, and all of that fits in a well-built questionnaire.

What cannot be skipped is the doctor. Sildenafil is prescription-only because it interacts dangerously with nitrates, and because erectile dysfunction is, in a proportion of cases, the first visible sign of a cardiovascular problem worth catching. The prescription is not paperwork — it is the filter that catches both.

How to get the prescription online, step by step

  1. Complete a medical questionnaire (2-3 minutes): cardiovascular history, current medication (nitrates above all), blood pressure, diabetes and how long you have noticed the problem.
  2. An EU-licensed doctor reviews your case individually. Average turnaround is under 4 hours, and always within 24.
  3. You get a decision. If approved, the electronic prescription lands in your account and you get an email. If declined, the doctor explains why in writing and your fee is refunded.
  4. You collect at a pharmacy and pay for the medicine there. Hi-Doctor does not sell, dispense or ship medication.

One detail on the pharmacy price saves real money and most people do not know it: Viagra is the brand; the active ingredient is sildenafil, and its patent expired years ago. Generic sildenafil contains exactly the same molecule at the same dose, under the same quality controls, at a fraction of the branded price. If the prescription is written by active ingredient, the pharmacy can dispense the generic — worth asking at the counter.

Neither Viagra nor generic sildenafil for erectile dysfunction is reimbursed by Spain's public system except in narrow circumstances, so in practice the medicine is paid in full either way.

Can you buy Viagra without a prescription in Spain?

No. Sildenafil is prescription-only, and Royal Legislative Decree 1/2015 bans "the sale, by correspondence and by electronic means, of medicines and medical devices subject to prescription". Any website offering it without a prescription is operating outside the law.

This is also one of the most counterfeited medicines in the world. Tablets circulating outside the legal chain have turned up with wrong doses, with undeclared active ingredients and with contaminants. The risk here is doubled: the counterfeiting problem stacks on top of taking a vasodilator without anyone having checked whether you take nitrates — a combination that can cause a dangerous drop in blood pressure.

Who qualifies: what the doctor assesses

CriterionWhat is assessed
Nitrates and nitric-oxide donorsAbsolute contraindication. Nitroglycerin, isosorbide mononitrate or poppers with sildenafil can cause severe hypotension.
Cardiovascular historyRecent heart attack or stroke, unstable angina, serious arrhythmia or advanced heart failure.
Blood pressureHypotension, or uncontrolled hypertension.
Current medicationAlpha-blockers and some antifungals or antiretrovirals may need precautions or a dose adjustment. Sildenafil with ritonavir is not recommended; with riociguat it is contraindicated.
Eye conditionsRetinitis pigmentosa and a history of non-arteritic anterior ischaemic optic neuropathy.
Liver and kidney functionSevere liver impairment contraindicates sildenafil. Other liver or kidney impairment may require a dose adjustment.

Worth not skipping: if the problem has come on gradually alongside other factors — high blood pressure, diabetes, cholesterol, smoking — the doctor may recommend a cardiovascular check as well as treatment. That is not bureaucracy; it is the reason this symptom deserves a consultation rather than a purchase.

The difference that changes the experience most is duration. Sildenafil means planning the dose with some lead time; tadalafil covers a far longer window, which is why many people prefer it. Which one fits depends on frequency, tolerance and your other medication — a decision the doctor makes with you, not a table.

What it does, and what it does not

Sildenafil inhibits the PDE5 enzyme and relaxes the smooth muscle in the blood vessels of the penis, letting blood flow normally during arousal. That last word is the point: during arousal. It does not produce an erection by itself, it does not increase desire, and it does not improve performance in someone with no problem.

It also does not treat the cause. Sildenafil may improve erectile function, but it does not work for everyone and it does not replace assessing and treating the underlying cause. If anxiety, a relationship issue, a medication that causes it or an undiagnosed vascular condition sits behind it, that is exactly why having a doctor look at the whole picture is useful.

Side effects and when to stop

The common ones are mild and short-lived: headache, flushing, nasal congestion, indigestion and sometimes a temporary change in colour vision or light sensitivity.

Some signals need urgent medical attention: an erection lasting four hours or longer, even without pain (priapism), sudden loss of vision or hearing, chest pain during sexual activity, or fainting. Do not wait on any of those.

Throughout treatment you can message your doctor from the private chat in your account. Every clinical conversation stays there, in an auditable record — no WhatsApp, no SMS.

Using the prescription at a pharmacy

The prescription is a Spanish private electronic prescription, digitally signed, delivered as a PDF in your account and by email. Your consultation is private; at the pharmacy the packaging identifies the product, and the pharmacist may ask what they need to dispense it safely.

In Spain, the pharmacy scans the QR code, validates it against the central registry and dispenses the product on the prescription.

Elsewhere in the EU, the prescription carries the elements Commission Implementing Directive 2012/52/EU requires for cross-border prescriptions: patient identification, prescriber identification and country, and the medicine by its international nonproprietary name. Directive 2011/24/EU requires each Member State to recognise a prescription issued in another Member State for a medicine authorised on its territory, and the pharmacist dispenses the locally marketed equivalent of the same active ingredient. Recognition is not automatic or guaranteed — the pharmacist keeps the right to refuse.
