Oral tenofovir disoproxil/emtricitabine PrEP has a well-established safety profile, but it is still prescription medicine. Baseline and follow-up checks are designed to find uncommon problems early, not to suggest that harm is expected.

Common early effects

Some people report nausea, loose stools, headache, tiredness or abdominal discomfort when starting. These effects are often mild and settle, but persistent vomiting or difficulty taking doses needs advice. The exact leaflet lists the frequency and full range of effects for the dispensed product.

Kidney monitoring

Tenofovir disoproxil is cleared through the kidneys and can rarely reduce kidney function or affect the renal tubules. Bone considerations

Do not add high-dose supplements without checking; exercise, nutrition and choosing the right PrEP formulation or pathway are more nuanced.

When to seek help

PrEP must not be started or restarted without excluding HIV infection at the appropriate time. Fever, rash, sore throat, swollen glands or a flu-like illness after a recent exposure may be signs of acute HIV and need prompt testing. A very recent high-risk exposure may call for urgent post-exposure prophylaxis instead; PEP is time-sensitive and should be assessed immediately.
