Alprostadil safety summary
Alprostadil cream is a topical treatment for erectile dysfunction: applied to the tip of the penis, it relaxes the local blood vessels directly so blood can flow in. Because it works locally rather than through the whole body, it is an option when PDE5 tablets are unsuitable or not tolerated. Most of its side effects are local too.

What it's used for
- Apply the single-use dose to the opening at the tip of the penis 5–30 minutes before sex, following the leaflet's technique.
- The effect lasts around 1–2 hours.
- Maximum one dose per 24 hours, and no more than 2–3 doses per week.
- Wash hands after applying.
Most common side effects
- Burning, tingling, pain or redness of the penis where the cream is applied.
- A mild rash or itching locally.
- Your partner may notice mild vaginal burning or itching after sex.
- Light-headedness shortly after applying.
Serious side effects: stop and seek care
- An erection lasting more than 4 hours (priapism): go to emergency care.
- Severe or persistent penile pain, or a new curvature, lump or hardening of the penis.
- Fainting or marked dizziness: the medicine can lower blood pressure in some men.
- A severe local or allergic reaction (spreading rash, swelling, difficulty breathing).
Who should not take it
- Conditions that predispose to prolonged erections: sickle cell disease or trait, leukaemia, multiple myeloma.
- An abnormally shaped penis, a penile implant, or severe scarring (Peyronie's disease): discuss with the doctor first.
- Men for whom sexual activity is inadvisable, for example because of severe heart disease.
- Allergy to alprostadil or any ingredient of the cream.
- If your partner is pregnant or breastfeeding, use a condom during sex.
Interactions to flag
- Other erectile-dysfunction treatments, including PDE5 tablets: do not combine.
- Blood-pressure medicines: the dizziness can add up.
- The cream can weaken latex condoms. Ask your doctor about compatibility if you rely on them.
- Anticoagulants: mention them to your doctor; any unusual bleeding at the application site should be reported.
